Breast Reconstruction After Mastectomy: Exploring Your Options and Timing

Undergoing a mastectomy is a significant and often overwhelming experience, but for many, breast reconstruction offers a way to restore their sense of normalcy and body image. The decision to reconstruct and the method you choose is deeply personal and should be made after considering various factors and consulting with your healthcare team. Here, I explore different breast reconstruction options, their pros and cons, and the timing of reconstruction to help guide you through this journey based on my personal experience.

 Also, remember that no matter what route you go, reconstruction is usually a multi-step process, and you may need multiple surgeries to achieve the best results.

Breast Reconstruction Options

Breast reconstruction options

1. Single Mastectomy with Implant-Based Reconstruction 

Advantages

  • Easier recovery compared to using your own tissue.
  • No delay in cancer treatment as complications from the healthy side are avoided.
  • Preservation of sensation in the healthy breast.

Disadvantages

  • Continuous future screenings of the healthy breast, which can be anxiety-inducing.
  • Difficult to achieve symmetry between the natural and reconstructed breast.
  • Potential for implant-related issues like infection and capsular contracture.
  • Implants need to be replaced over time, leading to additional surgeries.
  • Possible visible rippling of the implant due to minimal remaining tissue.
  • Generally less natural-looking compared to flap-based reconstructions.

2. Double Mastectomy with Implant-Based Reconstruction

Advantages

  • Easier recovery compared to using your own tissue.
  • Eliminates the need for regular mammograms, reducing anxiety.
  • Easier to achieve symmetry between the reconstructed breasts.

Disadvantages

  • Similar implant-related issues as with single mastectomy.
  • Loss of sensation in both breasts.
  • Less natural appearance compared to flap-based reconstructions.
  • Longer surgery and increased risk of complications and healing issues.
  • Possible delay in cancer treatment due to complications from the healthy side.

3. Single Mastectomy with Flap-Based Reconstruction

Advantages

  • Long-lasting results with no need for future upkeep.
  • Reduced risk of implant-related complications.
  • More natural look and feel.
  • No foreign objects in the body.
  • Easier to match a natural breast.
  • Potential for regaining sensation.
  • Possible improvement in body shape from the tissue donor site.
  • No delay in cancer treatment due to complications from the healthy side.
  • Potential improvement in lymphedema issues.
  •  

Disadvantages

  • Ongoing screenings of the healthy breast, which can cause anxiety.
  • Challenging to achieve symmetry with the natural breast.
  • Longer surgery and recovery time compared to implant-based reconstruction.
  • Risk of flap failure which: flaps won’t reestablish blood supply once connected. Ask your surgeon for their flap failure rate and how often they perform that kind of surgery
  •  Requiring a skilled microsurgeon (your Plastic Surgeon will need an additional skillset called Microsurgery which may be harder to find in your area), especially if you do not have much tissue and need a “less common” flap tissue reconstruction

Advantages

  • Long-lasting results with no need for future upkeep.
  • Eliminates implant-related risks.
  • More natural appearance and feel.
  • No foreign objects in the body.
  • Easier to achieve symmetry between reconstructed breasts.

  • Longer and more complex surgery with extended recovery time.
  • Greater need for tissue, which can be a challenge for slimmer individuals.
  • Risk of flap failure, requiring a skilled microsurgeon.
  • Possible delay in cancer treatment due to complications from the healthy side.

Timing of Breast Reconstruction

Your reconstruction can be immediate (at the time of your mastectomy) or delayed (weeks, months, or even years later). Remember, even if you choose an implant-based reconstruction, you can always later get the implants removed and have reconstruction with your own tissue.

 

 Immediate Reconstruction 

Advantages

  • Reduces the emotional impact of losing a breast.
  • May result in a better aesthetic outcome since the skin and tissues are preserved during mastectomy.

Disadvantages

  • Less time to consider all reconstruction options.
  • Additional treatments like radiation can complicate the reconstruction.
  • Potential anxiety and pressure to make a quick decision and finding a skilled surgeon.

 

 Delayed Reconstruction 

Advantages

  • More time to consider and plan the best reconstruction option.
  • Avoids complications from additional treatments like radiation.
  • Allows time to recover strength after initial cancer treatments.

Disadvantages

  • Requires an additional surgery and recovery period.
  • Prolongs the overall treatment timeline.
